This happens when you have a log entry with both then IN= and OUT=
interface (which mostly happens with forward rules). I consider this a
feature, not a bug. ;-)
(Most of the time and with other firewalls, only one interface is logged.
So I can only use one field for it. Instead of dropping one of the IN= or
OUT= interfaces with Linux iptables, they are concatenated if they are
both there.)
You can grep your log for "IN=eth0 OUT=eth0" to find those entries.
